Bottle Orders – Sampling Supplies
EMT provides required sampling containers free of charge, in most instances, to our clients. [We charge a nominal fee for Method 5035 sampling kits.] Bottles will either be shipped via Fed-Ex or other carrier at the client’s expense. Contact EMT with your project specifics, including the number and type of samples to be taken and the analysis to be requested. We will provide the appropriate bottles with preservative (as necessary), labels, shipping container and chain of custody forms.

Submitting your sample or product to EMT for analysis:
1. Download a copy of an EMT Chain of Custody Forms...Click Here to Download.
2. Call for pickup at 800-246-0633
or send the completed chain of custody form with your sample/product and a P.O. number or credit card number to EMT.
3. You will receive your analysis within the time promised. Sample results can be sent electronically to locations worldwide.

Courier Service
EMT provides sample pick-up services at your Chicago area office (within 50 miles) or project site seven days per week. Sample pick-up technicians can also deliver sampling supplies such as sample bottles and jars, preservatives, coolers and chain-of-custody forms to facilitate sampling efforts in the field. We provide prompt, efficient service using experienced environmental technicians.
